Patient Transfer for Physicians
If you are a provider and have a patient who needs comprehensive care for stroke, intricate surgery including cranial (neurosurgery) and spine (neurosurgery/orthopedics), vascular surgery, cardiac events, or complex pulmonary treatments, we are standing by to help your patient get the best possible care. For Providers
Please call UConn-MD Patient Transfer at 860-679-5555 and press 1 for the acute inter-hospital transfers. You will be immediately connected to our case manager or nursing supervisor to initiate the transfer.
When you call, please provide the following information so we can determine how we can best help you and your patient:
- Referring hospital
- Referring physician name
- Referring physician phone number
- Patient name and date of birth
- Reason for the transfer
- Type of specialty care or service requested
If you are a patient or family member of a patient, please have your physician make the transfer request. We are unable to accept transfer requests from patients and family members at this time.
